Identity Check

This App lists names and other key identifying personal, demographic and clinical details of all patients in any selected cohort. It allows you to produce filterable, sortable lists for many everyday purposes.

For example:

  • List a clinician’s current caseload, including key demographic and clinical characteristics.
  • List all patients who attended a particular clinic last week or last year.
  • List recent inpatient admissions including who admitted them, reason for admission/visit and length of stay.

Its features allow you to:

  • Select any data items of interest for inclusion.
  • Filter the outputs using selection criteria that you have chosen yourself.
  • Identify and create subgroups from the lists you have created.
  • Create averages for any numeric items, e.g. age, length of stay.

In the case of data items that may have changed over time, for example marital status or risk status, the report lists the most recently recorded answer.
