Key-Item Care Plans

Key-Item based Care Planning is the process of setting Goals and Interventions based on the Needs identified by Key Items highlighted during an Assessment, and then planning how to meet those Goals with the service user.

Care Partner allows you to create a Care Plan for service users and easily assign Goals and Interventions to meet their Needs, with the ability to add Contact Records and Care Plan Reviews. In order to create a Care Plan, a Key Item must be added within an Activity. See the Key items linked to activities section for more information.

Any field from an Activity marked as a Key Item within the service user’s record will pull through into the Care Plan.

You can only start a Care Plan if there is an Open Involvement, with Key Items highlighted in an Activity.
