Starting a New Form
There are two ways to start a new form in the system, depending on whether the person has had a form completed previously or not.
If the person has not previously had a form, or you are not sure, you can click New Form at the top of the Case List.
If you know that the person already has had a form completed, you can look for their identifier in one of the Case Lists. There will be a + icon in-line with their identifier.
You will then either be asked to choose the form you wish to complete, or the form will be opened automatically for you. This will depend on whether your system has one form setup or multiple - for example, if you have different forms available for Adults and Children’s services.
The form will then load the first section, where an Identifier is entered (which will link the form to either a new individual or an individual who already has forms on the system).
If you’ve opened the form using the New Form button, the Identifier field will be blank. You can start typing in this field.
If you’ve opened the page using the + icon for an existing individual on the Case List,the Identifier will be pre-populated with the Identifier for the existing individual chosen.
The format of your identifier will have been defined by your organisation, but it usually matches an identifier that you may use for individuals in your main local IT system.
Once you start typing, the system will check whether the Identifier already exists. If it does, select the correct Identifier and the form will be linked.
If the Identifier doesn’t already exist, select “Add” from the drop-down.
If your system has multiple Service Areas, you will be asked to select the appropriate one from the list.
A Service Area might be a team or a department - for example, if you have different localities using the same system, or more than one Formulate model.
If your organisation only has one Service Area available, this will be automatically filled-in for you.
Once these initial details are filled-in, the form questions will load.
