Form Selection

The next stage allows you to pick the forms that you want to include in your package.

Click on a form to include it in your package. You can use the Form Version column to opt to use an earlier version of a form.

Use the column headers to sort by form name, static ID, or form type, and use the Filter box.

A Patient registration type form must be included in the form selection. Only one patient registration form can be chosen.

Click Next to continue.

Gender Question

On the next screen, confirm which question on the chosen Patient registration form aligns to patient Gender. This allows you to make use of the Mortality Report within Atmolytics.

Click Next and confirm that the Gender answer options are correct.
