Form Types

When creating a form, you will be asked to select a Form type.

    Activity Records, Assessments and Contact Records are the activity types selectable within your Imosphere data entry solution when adding an activity.

  2. 2

    An Other Personal Details form is used for adding additional personal information in your Imosphere data entry solution from the Personal Details section. There can only be one OPD form per your Imosphere data entry solution instance.

    A Patient registration form is used for patient level forms for Atmolytics. Please note: this form type is not available for Imosphere’s data entry solutions as the registration/enrolment form is static within those system.

  4. 4

    A Subform is used to create repeatable sections.

  5. 5

    A Coding System is used for structured clinical coding hierarchies for Atmolytics. Please note: This is not applicable for Imosphere’s data entry solutions as these are managed separately.
